Medical Management  Assistance

  • Sole self-administration of medications by the resident - resident will be responsible for managing, administering, ordering, and refilling their own medications, including narcotics and all OTC's.

  • Screening assessment to be completed by the RN / LPN of The Legacy Place for Tier 0  approval.

  • Resident is responsible to provide their medication list and dosages to medical providers and facilities when needed.

  • Resident is responsible to schedule and arrange their own appointments as needed.

  • Resident will store, secure, and access their medications in the provided lockbox with exclusive key access managed by the resident.

  • RN / LPN of The Legacy Place can require a re-evaluation and assessment at any point to be completed by our facility RN/LPN when a concern arises that the resident may no longer be capable of self-administering or managing their medications.

  • Tier 1 Resident is not on any prescription medications. 

  • Tier 1 Resident's OTC medications will be locked with key access by the facility Med Aid.

  • Screening assessment to be completed by the RN / LPN of The Legacy Place for Tier 1 approval.

  • Resident is responsible for providing medications.

  • The facility nursing staff will order / refill medications, adjust or omit medications as prescribed by the resident's provider as needed.

  • Direct communication with the resident's healthcare professional regarding needs, concerns, or changes.

  • RN / LPN of The Legacy Place can require a re-evaluation and assessment at any point to be completed by our facility RN / LPN when a concern arises that the resident may no longer be capable of self-administering by medication planner / safe packaging or bubble pack set up.

  • Screening assessment to be completed by the RN / LPN of The Legacy Place for Tier 2 approval.

  • Resident is responsible for providing medications.

  • The facility nursing staff will order / refill medications, adjust or omit medications as prescribed by the resident's provider as needed.

  • Direct communication with the resident's healthcare professional regarding needs, concerns, or changes.

  • Tier 2 Medication set up via planner / safe packaging or bubble pack - set up by the facility RN / LPN and medication reminders if desired / needed.

  • Tier 2 Resident will store, secure and access their medications in the provided lockbox with exclusive key access managed by the resident.

  • RN / LPN of The Legacy Place can require a re-evaluation and assessment at any point to be completed by our facility RN / LPN when a concern arises that the resident may no longer be capable of self-administering by medication planner / safe packaging or bubble pack set up.

  • The facility designated Medication Assistant (MA I), nurse or physician will be responsible for passing all medications, including narcotics and OTC’s to the resident.

  • Resident is responsible for providing medications.

  • The facility nursing staff will order / refill medications, adjust or omit medications as prescribed by the resident's provider as needed.

  • Direct communication with the resident's healthcare professional regarding needs, concerns, or changes.

  • All medication are kept in a locked medication cabinet within the resident's unit or suite, except narcotics, which will be kept double-locked in the nurse’s station. 

  • The Legacy Place staff will retain keys to access the resident’s medications

  • A medication record (log) will be kept in the resident’s chart, which will be provided to the resident when being seen for medical reasons.

  • Tier 4 Medical Management is designated when their are four or more daily medication passes or when medication required extensive time or services.
