Medication Assistance/Management Options ~

(Our facility values that each resident's needs are different and we feel empowering independenace when possible and safe to do so is another way to allow our residents to continue to thrive within our facility.  The following options are offered with either the Senior Living or Assisted Living packages based on the initial Legacy Place nursing assessement)

Tier 1 -

* Sole Self-Administration of medications by the resident - resident will be responsible for managing, administering, ordering and refilling their own medications, including narcotics and all OTC's.   Required local primary care physician and The Legacy Place director of nursing written approval.

* Resident is responsible to provide their medication list and dosages to medical providers and facilities when needed.

* Resident is responsible to schedule and arrange their own appointments as neeeded.

* Resident will store and secure their medications in the provided lock boxe with exclusive key access managed by the resident.

* DON/RN of The Legacy Place can require a re-evalutation and assessment at any point to be completed by the resident's primary care physician, when a concern arises that the resident may no longer be capable of self-administering or managing their medications.

 

Tier 2 -

* Medication set up via planner/safe packaging - set up by the facility RN/LPN and medication reminders if desired/needed.  Required local primary care physician and The Legacy Place director of nursing written approval.  Final determination is on The Legacy Place's director of nursing.

* Resident is responsible for providing medications.

* The facility nursing staff will order/refill medications, adjust or omit medications as prescribed by the residents provider as needed.  

* Direct communication with the resident's healthcare professional regarding needs, concerns or changes.

* Resident will store, and secure their medications in the provided lock box with exclusive key access managed by the resident.

* DON/RN of The Legacy Place can require a re-evaluation and assessment at any point to be completed by the resident's primary care physician, when a concern arises that the resident may no longer be capable of self-administering by medication planner/safe packageing set up.

 

Tier 3 -

* Complete medication administration - Safe packaging set up required, hands on medication administration by a certified MA1.  

* Facility nursing staff will be responsible for storing and securing resident's medications, ordering/refilling medications, adjusting medication, dosages or delivery times including narcotics and OTC's.

* Direct communication with the resident's healthcare professional regarding needs, concerns or changes.
